The mission of the Primary Care & Hope Clinic is to improve the health and well-being of the communities we serve by providing excellent and compassionate care, accessible to all.

SERVICES - HOW WE DO IT

Primary Care & Hope Clinic serves as a medical home for uninsured and underserved patients in Middle Tennessee, regardless of their ability to pay. Through the consistent delivery of preventative, acute, chronic, and behavioral health services, the clinic ensures patients receive the care they need to get healthy and stay healthy. Primary Care & Hope Clinic is staffed by physicians, certified nurse practitioners, behavioral health consultants, and a registered dietitian.
